- Legal name of the agency and proof of 501C3 status
- Length of time agency has existed as a 501C3
- Agency's annual budget
- Primary mission of the agency
- Project overview
- Project goals, objectives, and expected results
- Amount requested
- Details of proposed budget
- Other funding sources for the project
- Signature of agency's chief executive officer or representative
You will receive an initial response letter indicating that either your request will be reviewed by our Grant Screening Committee or that your request has been declined. Once the Grant Screening Committee has reviewed your request, you will receive a letter either approving or denying the request or asking for additional information.
